RIGHTS AND PERMISSIONS GUIDELINES
Please be sure
to:
- Request
permission in writing to the appropriate contact
- Provide full
title, author, and publisher of article, table, figure, and page
number to be reprinted
- Provide purpose
for reprint, how material will be used
- Include name of
publisher and publisher's contact information, name of publication it
will be used in and date of anticipated publication
- Provide photocopy
of material to be reprinted
